What Is Source?
SOURCE, or Service Options Using Resources in Community Environments, is a state program designed to provide elderly, disabled and low-income patients with a wide variety of long-term health services, either in their home or community. This program is designed to decrease or minimize nursing home placement and unnecessary hospital stays.
